Dry Socket

Pathogenesis
๐Ÿ”น Due to fibrinolysis of clot
๐Ÿ”น Plasmin dissolves fibrin clot
๐Ÿ”น Severe pain + foul odor, but usually no pus

Risk factors
๐Ÿ”น Traumatic/difficult extraction
๐Ÿ”น Especially impacted mandibular 3rd molar
๐Ÿ”น Women, smokers
๐Ÿ”น Oral contraceptives (estrogen โ†‘ fibrinolysis)
๐Ÿ”น Also seen with Paget disease or post-radiotherapy

Clinical features
๐Ÿ”น Starts day 2โ€“3 after extraction
๐Ÿ”น Lasts about 7โ€“10 days
๐Ÿ”น Severe throbbing pain radiating to ear/neck
๐Ÿ”น Bad odor / bad taste
๐Ÿ”น Empty dry socket with exposed necrotic bone
๐Ÿ”น May have low-grade fever and ipsilateral nodes

Management
๐Ÿ”น Palliative only
๐Ÿ”น Gentle irrigation

Prevention

And even "natural" gum isnโ€™t safe.

Chewing gum, long considered a harmless habit, may be a hidden source of microplastic exposure.

A pilot study presented at the American Chemical Societyโ€™s Spring 2025 meeting found that both synthetic and natural gums can release hundreds to thousands of microplastic particles into saliva during chewing. Researchers from UCLA discovered that a single piece of gum could shed up to 3,000 particlesโ€”mostly polyethylene and polystyreneโ€”dislodged not by digestion but by the repetitive mechanical action of chewing. Surprisingly, natural gums offered no advantage over synthetic brands, with both releasing similar amounts of microplastics.

Led by environmental engineer Professor Sanjay Mohanty, the research adds chewing gum to the growing list of everyday items contributing to microplastic consumption. While scientists are still investigating the long-term health effects of ingesting these tiny plastic fragments, Mohanty advises reducing exposure where possibleโ€”such as chewing each piece longer instead of frequently switching to new ones. As microplastics continue to show up in everything from bottled water to household dust, this discovery serves as a reminder that even small, routine habits may have unseen consequences.
