Dear White N' Clean Dental Clinic Patients-Manila:
We regret to inform you that we had to permanently closed our clinic in Manila because of loss of income as a result of no operation since March 17-September 30, 2020.
The clinic management had to do a very painful decision to vacate the former dental clinic premises located at 922 Loyola Street, Sampaloc, Manila on September 30, 2020.
We are doing everything we can to continue your treatment with our contracted dental clinics while we are still recovering. Our contracted dental clinics are:

Requirements for dental appointments:
1. Must fill-up Contact Tracing Form
2. Wait for the advice on appointment availability
3. Once appointment slots are available, you will be notified to submit your RT-PCR Swab Test Result or Saliva Test Result thru text message. You will be given enough time to undergo a covid test. Take note that RAPID TEST is STRICTLY NOT ALLOWED. Covid Test must be done within 24 hours from the receipt of advice, otherwise, available date given may be forfeited. Test Result must be submitted within 24 hours from the date of the test result. Centrix Dental and Orthodontic Clinic reserves the right to not accept test result beyond the 24 hours allowance.
4. You will also be advised to isolate before and after the test.
5. Once sent, you will immediately advise to set an appointment within 24 hours.
6. Included in the advisory are the entry procedures to follow, treatment protocols and payment methods.
