This was such a difficult and challenging case, with the crown deeply embedded and positioned lingually, while the apex was located buccally with intimate nerve draping near the mental foramen.

CBCT-guided planning was essential due to the high risk of nerve injury. The tooth was carefully removed using combined buccal and lingual access, gradual sectioning, and controlled bone removal to minimize trauma and protect the surrounding vital structures.

But with prayers, patience, and meticulous surgical technique, we were able to remove the tooth safely and completely.

Grateful for the good tissue healing, but even more for the return of patient’s normal eating. Patient is finally gaining weight again. ☺️

Note: Patient was gently reminded again of the importance of good oral hygiene to avoid recurrence, & was advised to go back to her referring dentist for oral prophylaxis.

What starts as something as simple as missed dental cleanings can progress into something far more serious.

This case highlights how neglecting basic dental care such as oral prophylaxis can lead to chronic periodontitis, significant bone loss, tooth mobility, and eventually the development of reactive lesions like this large gingival growth.

Plaque and bacteria don’t just stay on the surface; they trigger chronic inflammation that affects both soft tissue and bone.

Routine dental visits and proper oral hygiene are not just for aesthetics—they are essential for preventing complex conditions that may eventually require surgical intervention.

Remember, oral health is deeply connected to overall health; what we ignore today can become a bigger problem tomorrow.

From high risk to manageable.

Proof that patience and proper planning matter. 🫶🏻Following incisional biopsy and marsupialization we did last Oct 2, 2024, this lesion in our 72-year-old patient dramatically reduced in size, therefore minimizing surgical risk and making definitive enucleation safer and more predictable. 🙏🏻
