This was such a difficult and challenging case, with the crown deeply embedded and positioned lingually, while the apex was located buccally with intimate nerve draping near the mental foramen.
CBCT-guided planning was essential due to the high risk of nerve injury. The tooth was carefully removed using combined buccal and lingual access, gradual sectioning, and controlled bone removal to minimize trauma and protect the surrounding vital structures.
But with prayers, patience, and meticulous surgical technique, we were able to remove the tooth safely and completely.
