One thing I relish about my job is to get to know my patients over the years and to see how my dental work fared.

I first met this 16 year old teenager when she was referred for RCT 37 by a GDP, who initiated the RCT but met some difficulty. The tooth had a wavy C-shaped root configuration.

3 years later, she was referred back to me for restoration and RCT of her 27, which was equally wavy. This teenager had since turned into a pretty young lady.

We reviewed her 3 months following her RCT 27 and noted resolution of periapical radiolucency. We agreed to provide cuspal coverage indirect restoration to increase fracture resistance of both the root-treated 27 and 37.

Proximal caries is tooth decay that develops on the touching side surfaces (mesial or distal) between adjacent teeth, typically just below the contact point.

It occurs where two teeth touch, making it hard for toothbrush bristles to reach. It is also hard to spot with naked eye, so radiographs (esp. bitewings) are often necessary for diagnosis.

This patient attended complaining of sensitivity at his lower left quadrant. Proximal caries was diagnosed at both his lower left 1st (36) and 2nd molars (37) from x-ray.

Restoration of 36 involved dismantling the old crown, restoring the tooth with composite and redoing a new crown after a period of observation. 37 was restored with composite filling.

Just when we thought all are sorted,the patient reported unprovoked pain at 37.Vital pulp therapy was carried out for 37, after removing the inflamed pulp tissue from the pulp chamber and achieving haemostasis for the radicular pulp, to err on the side of conservation of tooth tissue.

Your dentist may recommend root canal retreatment before placing a permanent crown on your tooth, although you have no symptoms on the tooth.

This is to ensure a sound foundation before placing the permanent crown, which can be costly.

Besides, if an infection flares up after you get the new crown, the dentist might have to drill a hole through your brand-new crown (essentially a blind procedure) or remove the crown entirely. This would cost you even more, not only financially, but also biologically (unnecessary loss of tooth structure).

A root canal treatment (RCT) is a dental procedure to remove inflamed or infected pulp on the inside of the tooth which is then carefully cleaned and disinfected, then filled and sealed.

RCT is designed to eliminate bacteria from the infected root canal, prevent reinfection of the tooth and save the natural tooth (AAE).

A diagnosis conundrum..

This pleasant lady had few treatment with us on her upper right quadrant. She developed right facial swelling in between waiting for her crowns. We suspected her upper right 2nd premolar (15) to be the cause but the tooth tested vital to every mode of pulp testing.

Shortly after, she developed another episode of localised swelling at the same tooth. We managed to insert a gutta percha and tracked the causative tooth. The tooth likewise tested positive to pulp testing.

Puzzled by the contradictory finding, we ordered a CBCT, only to find visible crack lines at the palatal root of her otherwise unrestored 15.

Therefore, we recommended extraction and replacement for her 15.
