🟢⛔️🟢 Immediate dentin sealing (IDS):
⛔️What is it ?!!!!!
The Exposed dentin immediately after preparation of vital tooth or teeth is or are susceptible to insult from bacterial infiltration and microleakage during the provisional
phase between impression taking and delivery ofthe final restoration.
🟢The pe*******on of bacteria and or fluids from the oral cavity through the open
dentinal tubules can result in colonization of
microorganisms, postoperative sensitivity, and the potential for subsequent irritation of the pulp which finally leads to irriversable pulpitis.
🟢So the Management of the dental tissues between the preparation and provisional phase of restorative treatment plays an essential role in the success of indirect adhesive restorations.
🟢The most effective way of managing and
protecting the ((pulp-dentin complex ))is through inhibition of this process to start by "immediatedentin sealing" directly after preparation and before any further step.
So The "immediate dentin sealing" technique allows the development of a hybrid layer on vital teeth immediately after cavity preparation.
🟢 This hybrid layer is described as a polymerized resin intermingled with collagen fibers.
🟢This procedure protects the pulp from mechanical trauma, thermal stimuli, and bacterial invasion.
⛔️🟢The Clinical Steps:
🟡 1A. For Total-Etch Method:
a. clean the prep with disinfectant .
b. Etch the dentin for 5 seconds only using a 37% phosphoric acid.
c. Rinse thoroughly with distilled water and remove excess
water by suction tip (( don’t over dry ))leaving preparation visibly moist.
d. Apply adhesive primer to moist prep, then gently air dry - Surface should appear shiny otherwise repeat process.
e. Light cure for 10 seconds.
🟡 1B. For self-etch method:
a. Clean prep with disinfectant.
b. Rinse thoroughly by distilled water & remove excess water & keep moist prep.
c. Apply self-etch adhesive, agitate, air dry,
d. Light cure for at least 10 seconds./
2. Remove the oxygen-inhibited layer of the freshly bonded surfaces with an alcohol moistened pellet.
3. Impression taking.
4. Apply separating medium and gently air dry for 10 to 15 seconds to evaporate solvent. (if there is no separating medium, let abutments wet during next step to avoid adhesion between temporary crown material and bonding material.
5. Fabricate provisional, finish,
6. Rinse separating medium off prep with water.
7. Cement provisional restoration.
8. At final cementation: remove provisional,
thoroughly clean the prep with plain pumice slurry (or air abrasion),
9. Proceed with adhesive protocol (acid-etch and adhesive under isolation).
10- cementation with resin cement is preferred.
