🦷 Tooth Extraction: When to Remove… and When to WAIT! 🚨
One of the biggest INBDE traps is thinking:
“Painful tooth = extract immediately.”

Not always.
The real exam question is usually asking:

👉 Is the tooth hopeless?
👉 Is the patient medically stable?
👉 Will extraction create a bigger complication?

Let’s simplify it.

✅ When is extraction INDICATED?

Extraction is usually indicated when the tooth is non-restorable, harmful, or strategically useless.

1️⃣ Non-restorable caries

If caries has destroyed the tooth so badly that it cannot be predictably restored, extraction becomes the correct choice.

Example:
A tooth with deep subgingival decay, poor ferrule, and poor crown-root ratio.

📌 INBDE trap:
Deep caries alone does NOT mean extraction.
If the tooth is restorable, think RCT + restoration, not extraction.

2️⃣ Hopeless periodontal prognosis

Extraction is indicated when there is:

✅ Severe mobility
✅ Advanced attachment loss
✅ Severe bone loss
✅ Poor crown-root ratio
✅ Recurrent periodontal abscess
✅ Hopeless furcation involvement

📌 Classic phrase:
“Hopeless periodontal prognosis” = extraction.

3️⃣ Failed endodontic prognosis

Extraction may be required when:

✅ RCT has failed
✅ Retreatment is not possible
✅ Apical surgery is not feasible
✅ Persistent periapical infection remains
✅ Tooth is structurally non-restorable

But remember:
A periapical lesion does not automatically mean extraction if the tooth can be saved.

4️⃣ Impacted tooth with pathology

Impacted teeth are removed when they cause problems such as:

✅ Pain
✅ Recurrent pericoronitis
✅ Cyst formation
✅ Caries on adjacent second molar
✅ Periodontal defect distal to second molar
✅ Root resorption of adjacent tooth

📌 INBDE trap:
An asymptomatic impacted third molar is not automatically extracted in every question. Look for pathology or risk.

5️⃣ Orthodontic or prosthetic planning

Extraction may be done to:

✅ Create orthodontic space
✅ Remove a tooth with poor strategic value
✅ Improve
