Different week…!! 🎄❄️☃️ Same goals 🙌🏻

This patient came in with

🎄 Severe crowding
🎄 Collapsed or deep bite
🎄 Transverse roll of the anterior occlusal plane
🎄 Lack of Incisal show during smile
🎄 UR 3 and LR 3 out of the arch
🎄 Narrow arches

There was no way to unravel this kind of severe crowding without extractions. Inspite of all 4 premolars extraction, significant expansion was achieved so that his buccal corridor space was not obliterated. Bite opening happened simultaneously during space closure with some reverse curve arch wires.

He finished with satisfactory Incisal display & a beautiful near normal overbite and overjet..!!!

A frenum that attaches too close to the gingival margin of a tooth can cause gingival detachment and resultant loss of bone, This patient’s chief concern was

🔹 Spacing and rotations between lower teeth
🔹 Upper centrals being too prominent
🔹 Mobility in the lower front teeth
🔹 Lisping and difficulty in articulating speech due to
restricting Lingual frenum

Treatment plan involved aligning and leveling followed by space closure and minimal IPR to close the black triangles and lingual frenectomy was performed to remove all of its remnants from the lingual salivary gland upto its intersection with the underside of the tongue. Good interincisal contact was achieved to ensure long term stability

Treatment duration: 5months.

This patient came in with concerns regarding

Save teeth when you can !

Patient preferred a bridge here instead of implants,In my opinion, bridge is a faster and a simpler solution to achieve a satisfactory result. If the foundation work is good, both implants and bridges have almost the same long life as they promise.

Only one or two of these teeth have been devitalized due to pulpal involvement, the rest are vital.
