What do you do when your last standing molar is shot 🥲
For this particular situation, I was worried about the distal friend end saddle being unstable on such a narrow ridge.
My patient had a complex medically history, so vertical and horizontal augmentation wasn’t a viable option (and patient wasn’t keen for this).
I chose to extract the LL6, which had a vertical root fracture, disinfect the socket and carry out ridge preservation. This was with the aim of maintaining the height and width of the socket for possible implant placement.
The plan was to see if I could get a short implant in that site to support the distal free end saddle.
Once the graft had healed I used SIAB (smile in a box ) to plan a digitally guided implant placement, involving my fantastic denture technician at Creative Chrome.
Access at the back of the mouth is tricky in these sites, and guide stabilisation was of utmost importance on that distal free end saddle. I used a strategically positioned stabilisation pin to help with this.
I was restricted with the length of my implant but was able to place a short 6mm implant to support a cobalt chrome denture.
The final denture framework was beautiful and fit so snuggly. The hygienic design was comfortable and retentive, meaning the patient was thrilled with her new reliable tooth replacement!
