How to remove old crowns efficiently?

Busy couple of days
Monday morning; 7 crown preps and impressions for 17 implant units (one patient)
This morning; 11 crown preps (one patient)

Removing old crowns efficiently is super important for these big cases, and there are ways to make sure you’re not spending forever doing it.
1. New burs. Don’t mess about with old dull burs, I got through 5-10 burs in each session (from packet to bin)
2. Diamond burs for porcelain
3. Tungsten carbide burs for metal

These 3 things will make a huge difference! Bonus point; electric speed increasing handpiece

I’m always shocked how much of a difference the cement makes when bonding ceramics.

Lithium disilicate adhesive onlay (A1) bonded with total etch technique, scotchbond universal and B1 gaenial universal injectable.

Such an honour to have come and spend the day at

He is less than one year out of dental school, and is a student within the fellowship of restorative dentistry.

He asked if he could come and shadow me, and, of course, I was more than happy to help out. I didn’t realise he would be driving over 4 hours each way to get to us!

It was a great day, lots of great questions, and fantastic to see such an eager dentist doing great things so early in their career. Big things to come from you Hamaad, I’m sure

We can use zirconia for veneers now!?

and I have had a few cases where they felt Zirconia was needed, but I wanted the benefit of adhesion (to minimise the amount of preparation needed).

The recent before and after I posted showed the aesthetic outcomes we’ve been able to achieve, and I’ll post the full case photos soon.

I don’t want to jinx it, but on the few cases we’ve done it so far, it’s worked well! I’d still prefer a glass ceramic restoration like emax with its long term evidence base, but I’ve moved from a ‘never’ to a ‘sometimes’.

This molar came out and the implant went in at the same appointment.

Immediate implants have really changed how I think about implant placement after 5 years of more early or delayed placements.
Less surgery for the patient.
More efficient for us.
And a powerful way to shape the emergence profile from day one.

Where I can immediate implants are becoming my first choice.

Straumann implant placed immediately after extraction.

Jump gap grafted with Bio-Oss Collagen and a custom healing abutment used to sculpt the tissue.

Of course this only works when primary stability is achievable. If not, I bury the implant and cover with Mucograft.

Really trying to get used to using the operating microscope

It’s great for vision, lighting and posture, but it also gives unrivalled content for documentation and teaching!
