RCT for a lower 2nd premolar with a deep apical split (KING of variations)
It’s very important to analyze the pre-operative radiograph very well, specially in tricky lower premolars and it preferred to have a view on CBCT
Steps: MANUAL GAME
1. coronal flaring
2. Scouting with a pre-bent #10 K file oriented lingually to catch the lingual canal entrance.
3. Shaping the buccal (wider) canal first.
4. Re-entering the lingual with #10 K file and creating a smooth glide path till #25 K file.
5. Using a pre-curved rotary files manually inserted into the lingual canal then attaching the rotary motor to them to accurately enter and shape the canal.
6. GP size 25/4 L, 30/4

‏Space maintainers are mandatory when baby teeth have been lost/extracted due to extensive decay way ahead of the time of eruption of the permanent ones.
‏They prevent the remaining teeth from tilting or drifting which avoids the need for braces later on and loss of the normal alignment or spaces within the arch.

حافظ مسافة الأسنان لا يُستخدم إلا في حالات معينة ويتم اللجوء إليه في حال سقوط أحد الأسنان اللبنية للطفل نتيجة تكون خُراج أسفلها مثلًا أو لأي سبب آخر أدى لسقوطها مبكرًا ولا يزال متبقي عام أو أكثر على بزوغ السنة الدائمة.

إذا لم يتم تركيب حافظ مسافة للأسنان في هذه الحالة، فإن ذلك يؤدي إلى تحرك الأسنان الدائمة المجاورة تجاه الفراغ الخاص بالأسنان اللبنية التي سقطت، الأمر الذي يؤدي إلى اعوجاجها عند نموها وربما تظل مدفونة داخل الفك لأنه امتلئ بالعظم، بالتالي يكون من الصعب اختراقه لتبدأ الأسنان في الظهور
