External Cervical Root Resorption (ECR)

Silent but potentially destructive lesion that begins at the cervical root surface, often with vital pulp and minimal symptoms.

Clues to suspect it 🗝️:

• Pinkish cervical discoloration ⭕
• Unusual cervical radiolucency ⁉️
• Positive vitality test ✔️
• CBCT revealing the true extent 💡

Management depends on the stage ⏳:

• Early lesions → surgical exposure, removal of resorptive tissue, and restoration (Bioceramics/RMGI/Composite).

• Pulpal involvement → RCT + defect repair.

• Advanced destruction → extraction ❌

Early diagnosis is the key to saving the tooth ✅

Not Every Good Endo Survives ❌

Sometimes the endodontic treatment looks perfect radiographically, yet the tooth is already compromised ❗

• Persistent pain on biting
• Isolated pocket
• Recurrent abscess

Extraction confirmed it: Vertical Root Fracture (VRF).
