A 62-year-old patient presented to our clinic complaining of severe maxillary atrophy. The patient reported a previous failed implant treatment performed eight years earlier in another clinic, which resulted in a negative overall experience.
A full skull CBCT scan was obtained for comprehensive evaluation. Radiographic assessment revealed extreme bone resorption in zones 2 and 3, according to the Bedrossian classification, with complete absence of sufficient bone. Due to this severe atrophy, pterygoid implants were not a viable option. The anterior maxillary region (zone 1) also exhibited severe horizontal bone deficiency, rendering conventional implant placement impossible.
Based on these findings, an advanced surgical treatment plan was established as follows:
• Placement of two bilateral zygomatic implants in the premolar regions
• Placement of two transnasal implants following careful reflection of the nasal mucosa
• Placement of one implant in the nasopalatine canal to help reduce and distribute cantilever forces resulting from the compromised bone support
The surgical procedure was performed successfully. One week postoperatively, the patient received a fixed temporary prosthesis through a denture conversion technique, ensuring immediate function and patient comfort. The prosthetic phase was carried out by the talented Dr. Mohamed Amr, whose expertise contributed significantly to the successful outcome.
