Surgical extraction of impacted lower wisdom.

A 25-year-old female patient presented to the dental clinic with severe pain on the right side of the jaw, extended to the ear, accompanied by noticeable swelling on the same side.
The patient was prescribed analgesics and antibiotics for 5 days, followed by a surgical extraction of the lower right third molar.

The patient previously had RCT on the lower 1ST molar, followed by an amalgam restoration.

After some time, the amalgam fractured, and noticeable loss of tooth structure involving the disto-lingual cusp.

We decided to remove the remaining amalgam and restore the tooth using composite resin to achieve both functional and aesthetic results.

Patient Summary

History:
• Dental trauma to upper central and right lateral incisors 5 years ago.
• Endodontic access, cleaning, and intracanal medication (calcium hydroxide) were performed 1 year ago but treatment was not completed.
• Patient recently reported numbness in the right infraorbital region.

Findings:
• Clinical: sinus tract, extensive coronal destruction.
• Radiographic: large periapical radiolucencies.

Diagnosis:
• Necrotic pulp with chronic apical periodontitis (teeth #11, #12, #21).

Treatment:
• Removal of old medication, thorough irrigation, new intracanal medication for 2 weeks.
• Final root canal treatment performed after resolution of symptoms.
•cervical seal

Plan:
• Follow-up every 3–6 months.
