January was about learning from leaders in orthognathic surgery at the Arnett & Gunson Forum, exploring Santa Barbara, San Luis Obispo, Morro Bay, taking the family to the San Francisco Zoo and to their first NBA game - Go Warriors!

SHARING KNOWLEDGE - Quick question…
Is maxillary fixation with two plates stable?
Short answer… YES - read to understand why and when to use or not to use it.

Clinical Research – JCMFS 2025
A prospective quasi-experimental study comparing the use of 2 plates or 4 plates to fix the maxilla in Orthognathic Surgery.

Key Findings
👉🏻Only patients with bimaxillary advancement and small vertical surgical movements - maximum 2 to 3 mm of maxillary downward; It means, no complex cases, just routine cases...
👉🏻FOLLOW UP of 1 to 3 years;
👉🏻3D assessment;
👉🏻COMPARATIVE study between 2 plates and 4 plates;
👉🏻NO DIFFERENCES in surgical stability between 2 or 4 plates;
👉🏻NO DIFFERENCES in surgical stability if perfomed counter-clockwise rotation or clockwise rotation;

🗣️My CLINICAL MESSAGE - My preference is to fix the maxilla with two pre-bent 2.0 plates, and this paper shows that some surgical movements are highly stable—usually those planned in our routine. However, it is very important to know when to change the strategy. Here is an expert tip: use four plates in cases of significant posterior maxillary expansion, total maxillary downward movement (with no bone contact), and when performing a maxilla-first approach.
..knowing when not to do is more useful than knowing when to do...
Don’t overdo it!!

🗣️Vi este slide durante a palestra da Dra. Yuri Agrawal no Annual Research Day do Departamento de OHNS de Stanford, e suas ideias definem a minha trajetória profissional. INSPIRAÇÃO PURA!!

Aprendi respeitando o conhecimento estabelecido, mas nunca o aceitei sem questionar.
É a mente inquieta que me conduz para fora da zona de conforto e me impulsiona a buscar respostas.

I learned by respecting established knowledge, but I never accepted it without questioning it.

- I participated in the Mexican Congress of Oral and Maxillofacial Surgery as a speaker - the family loved being in Cancun and I have a lot of fun with the friends!

- I took the opportunity to learn a little more from the associated specialties, especially Otolaryngology…

E, entre tudo isso, aprendendo cada vez mais sobre Distúrbios Respiratórios do Sono e suas abordagens — um capítulo inspirador nesta jornada em Stanford.

🗣️Discovering the Bay Area, celebrating Leo’s 5th birthday, welcoming the first visit from his godfather , enjoying the beginning of fall and Halloween…

And beyond all that, learning so much about Sleep Breathing Disorders and their approaches!

SHARING KNOWLEDGE - Reviewing some papers today, I remembered one that really changed my surgical decision-making when choosing which mandibular osteotomy to use in different cases.

🗣️Read until the end — the insight is worth it!

Clinical Research – IJOMS 2020
Bone Interferences between proximal and distal segments in BSSO

Study design - One hundred patients were virtually planned and then compared with the intraoperative results.
Purpose: verify if virtual planning matched the surgical reality — and when bone interferences appeared, understand how to manage them to avoid complications or TMJ disorders.

Key Findings
👉🏻VIRTUAL PLANNING is 100 % sensitivity, 51 % specificity. It means, if there was no interference in the virtual plan, there was no real interference; but if planning showed interference, it didn’t always occur in surgery;
👉🏻CLOCKWISE rotation of the mandible was the surgical movement with more Interferences and each 1 mm downward movement of B-point increased interference risk by 26 %.
🗣️My CLINICAL MESSAGE - I perform Posnick osteotomy for clockwise rotations without major advancements — typically in Class III cases needing clockwise rotation + setback.
👉🏻When bone interference occurred, the Ellis (2007) lingual osteotomy correction worked perfectly;
👉🏻The proximal segment behavior showed only ROLL rotation (4–5° on average), with no yaw/pitch rotation, no TMJ disorders or pseudarthrosis up to 1 year postop.
🗣️My CLINICAL MESSAGE - ROLL rotations up to 5° are tolerated — useful for improving intergonial width as facial aesthetic management.

A single paper, multiple insights — it truly changed something in my daily routine.
